Patients, parents, legal guardians, and third-party requestors often need access to medical records for continuity of care, legal documentation, or insurance purposes. Here’s everything you need to request your child’s medical records quickly and securely.

How to request medical records

Parents and legal guardians:

To request medical records that are not available on the patient portal, please contact your Healthy Kids Pediatric Group office. You can submit a signed authorization form by fax or in person. Other healthcare providers can also request that your child’s records be sent directly to their office with a completed authorization form.

Third-party requestors:

All third-party requestors must submit an Authorization for Release of Protected Health Information (available in English and Spanish). For patients under age 18, this form must include a parent's or legal representative's signature. In some cases, additional documents may be required.

Which records do you need?

Basic records
Basic records such as visit summaries, lab results, and immunization records are typically available for pickup at your Healthy Kids Pediatric Group office. These records are also available on the patient portal.

Entire chart
Requests for additional records or a full medical chart must go through your Healthy Kids Pediatric Group office. Fees and processing times vary depending on the request type and delivery method (in person vs. mailed).
