Our Staff at Healthy Kids Pediatric Group would like to welcome you and thank you for selecting us to provide your child’s medical care. As a new patient, you will naturally have many questions regarding the policies and practices of our pediatric office. Hopefully the information which follows will be helpful to you as we get to know each other.
How To Transfer Into Our Practice
Please use the forms below to begin the process of transferring into our practice. Please fill in the appropriate sections and send to your previous doctor. You may also print our registration and HIPAA forms and bring it to your first appointment in the office.
- Release of Medical Records

- HIPAA Form
- Financial Agreement
- Consent to Release Medical Information (to a parent)
Our patients, newborns through adolescents, are seen by appointment for illness, injury, and well child examinations. We respect the time spent in the waiting room and we make every effort to minimize any delay.
Emergencies do occur, however, and such patients are seen on a priority basis. You would want this accommodation for your child and accordingly we hope that you will understand if your scheduled visit should be delayed under such circumstances.
If you must cancel an appointment, please notify us 24 hours in advance. There is no charge for the first missed appointment, however, if there are repeated missed appointments, without prior cancellation, subsequent office visits will not be scheduled.
Payment at the time of the visit is requested. Fees are based on services rendered. If a financial hardship exists, an agreed upon realistic plan for payment will be considered.
Starting school or ready for sports season? Print the appropriate forms below, fill out the appropriate sections, and bring to our next physical.
Your health insurance coverage may or may not include office visits for illness, well child examinations, vaccines or hospital care for a normal newborn. In recent years, health insurance has dramatically changed and has become increasingly complicated for patients and physicians alike. We recommend that you familiarize yourself with your insurance benefits.
Like most other pediatric offices, Healthy Kids Pediatric Group, makes every reasonable attempt to work with both the insurance company and the patient. We are happy to assist you, for example, in completing forms so that you may be reimbursed for any funds to which you are entitled.
Although we have agreed to serve as providers for many insurance plans, the terms of the policies vary, even within a particular insurance program. It is, therefore, absolutely necessary that you understand exactly what your insurance covers and especially the requirements specific to your policy ( office co-pays, deductibles, eligible hospitals, referrals to specialists, emergency care, etc.)
We need your assistance and cooperation so that we can render the best care possible to your family.
Many mild illnesses and minor problems can be discussed over the telephone, thereby eliminating the need for an office visit. However, certain problems (for example eye injuries, skin rashes, questionable lacerations,) must be seen before a diagnosis can be made and appropriate treatment begun.
If you call during office hours please be patient. The doctors will return your call as soon as they are able. If you think your child needs to be seen, please do not wait for a return call. Make an appointment so that your child may be seen expeditiously. If your child is ill, please have your child’s temperature available. Antibiotic medications and antibiotic eye drops will not be prescribed over the telephone. If your call is urgent, please indicate this to the receptionist as we will interrupt the current appointment and speak to you immediately. If your call is not returned in a reasonable time please call again.
There is no charge for a brief telephone consultation.
Is the office closed? Someone is available 24 hours a day to answer your urgent questions. Just call our regular phone number at (609) 448-7300 and follow the prompts on the service message and we will get back to you as soon as possible. If you have not heard from us within 15-20 minutes please call again.
